• Dental injury
• Failure to return to pre-op, LOC
• Hyperthermia, temperature > 38.3 C
• Hypothermia, temperature < 35.0 C
• Seizure
• Unplanned patient awareness
• Allergic or anaphylactic reaction
• Inadequate reversal
• Narcotic excess requiring Naloxone
• Post-operative delirium requiring tranquilizers
• Transfusion reaction
• Post-intubation croup
• Soft tissue / naso-oropharyngeal injury
• Laryngospasm
• Pulmonary aspiration
• Pulmonary edema
• Cardiac / respiratory arrest
• Acute myocardial infarction
• Congestive heart failure
• Hypertension > 110 diastolic > 10 minutes
• Hypotension < 80 systolic > 10 minutes
• New PVC’s > 5 per minute
• New ST segment changes
• New non-sinus rhythm
• Sinus bradycardia < 40 beats per minute
• Sinus tachycardia > 10 minutes
• Corneal abrasion
• Wrong site surgery
• Arterial or central line complication
• Protracted vomiting
• Unplanned PACU stay over 2 hours
• Infiltrated IV catheter
• Undocumented burns, tears, lacerations, etc.
• Patient expired in PACU
