Epidural steroid injections

Epidural steroid injections:

• Use 80 mg of depomedrol (2 ml)
• Use 6 ml of 1/8% bupivacaine
• Total volume is 8 ml
• If patient has one-sided symptoms, the transforaminal approach is better and that requires fluoroscopy
• If patient has midline or bilateral symptoms, traditional translaminar approach is fine
• Bill Lyman uses 80 to 120 mg of depomedrol in 10 ml of normal saline. He doesn’t use bupivacaine. He gives three injections over a 3 month span.
